Health worker shortages, a government’s failure to be accountable, Dec 2010

Councils’ Human Resources Officers and President’s Office for Public Services Management (PoPSM) officials should look to their laurels by addressing council specific medical staff shortages that compromise quality and quantity of healthcare service delivery. They should closely collaborate with the Ministry of Health and Social Welfare (MoHSW) and Prime Minister’s Office Regional Administration and Local Governments (PMORALG) to employ new health workers as per annual Councils’ requests.

Following a recent visit to Temeke Hospital by the Minister for Health and Social Welfare, poor quality of health service delivery at the hospital and critical Staff shortages were discovered. While the MoHSW officials laid blame to the Municipal Director for not requesting enough health workers, media reports quoted Temeke Municipal Director, Steven Kongwa saying that in the fiscal year 2009/10, they requested for 250 health workers but only 20 were provided.

Following these contradictory reports, Sikika, a nongovernmental organization working to stimulate governance in the health sector, did analysis of human resource planning by looking at annual health plans for 32 councils IN Tanzania. This was followed up by anonymous interviews with few officials from various levels of government. Our findings indicate that various forms of problems lie at different levels of government.

Hon Maige’s ‘cleaning’ of ministry commendable, Dec 2010

The Minister of Natural Resources and Tourism, Ezekiel Maige’s move to ‘clean’ by investigating misdeeds committed in the ministry and imposing a ban on international trips is the right step in the direction of reducing unnecessary use of taxpayer’s money.
Hon Maige among other measures imposed a ban on international journeys by his ministry’s officials saying that they should remain in the country implementing their duties and instead visit our tourist attractions, before warning that stern measures would be taken against irresponsible officials who have been causing the government revenue to drop.

Actually, for the year 2010/11, the Ministry of Natural Resources and Tourism has allocated a total of Tshs 5,184,245,100 for travels out of which Tshs 666,115,000 is for out of country travels and Tshs 4,518,130,100 is for in country travels.

Rushwa hapa nchini, imejitokeza katika aina mbili; rushwa ndogondogo na rushwa kubwa. Rushwa ndogo ndogo imekuwa tishio kwa Serikali kwani zimeendelea kukithiri katika sekta ya afya. Hali hii siyo tu inawaathiri watu wenye kipato cha chini (wasio na uwezo wa kutoa hongo), bali pia inachafua taaluma ya udaktari nchini.

Hivi karibuni, Sikika imeandika kitabu kutokana na utafiti uliofanyika juu ya rushwa ndogondogo katika huduma za afya, kitabu hicho kinachojulikana kama “Rushwa Ndogo Ndogo katika Huduma za Afya Dar es Salaam na Pwani ” kinaonyesha jinsi rushwa inavyojitokeza katika utoaji wa huduma za afya katika wilaya nne za Kinondoni, Temeke, Ilala na Kibaha. Lengo hasa la utafiti huo ni kupima uelewa wa watumia huduma za afya katika rushwa na pia kujua dhima, uzoefu na mchango wao kwa katika kuendeleza vitendo vya rushwa na athari za rushwa kwa wanaotafuta huduma za afya.

Unnecessary expenditures to rise 537 bn Tsh by (2010/11)FY

Despite the insufficient funds for the national budget, there are items in this budget that are
considered unnecessary due to their perceived negligible utility to the majority of Tanzanians.
It is fortunate that the government has also recognized this problem of unnecessary expenditure
and has through its top officials like the Prime Minister Mizengo Pinda committed to increase
control over and economize those expenditures.
The sum of all unnecessary expenditures in the Ministries, Departments and Agencies (MDAs)
as well as regions fell from 684 billion (bn) Tanzanian Shillings (TSh) in financial year (FY)
2008/09 to 530 bn TSh in FY 2009/10, which is a reduction of 22.4 percent. But, for the FY
2010/11 these expenditures are expected to rise slightly to 537 bn TSh, a move which
contradicts the government’s stated commitment to spend its resources more wisely.

Matumizi yasiyo ya lazima kuongezeka kufika Tsh. 537bn kwa mwaka wa fedha 2010/11

Pamoja na ufinyu wa bajeti ya taifa, kuna baadhi ya vipengele katika bajeti hii ambavyo
vinaonekana kuwa havina tija kutokana kwamba haviwanufaishi watanzania walio wengi.
Ni bahati kwamba serikali pia inalitambua tatizo hili la matumizi yasiyo ya lazima kwa wananchi
walio wengi na kupitia viongozi wa juu wa Serikali kama Waziri Mkuu, Mh. Mizengo Pinda,
ilidhamiria kuongeza udhibiti wa matumizi hayo.
Jumla ya matumizi yote yasiyo ya lazima katika Wizara, Idara na Taasisi za Serikali yalipungua
kutoka shilingi za kitanzania bilioni 684 katika mwaka wa fedha wa 2008/09 hadi shilingi za
kitanzania bilioni 530 katika mwaka wa fedha 2009/10, ambapo ni punguzo la asilimia 22.4.
Lakini, kwa mwaka wa fedha 2010/11. Matumizi haya yasiyo ya lazima yanategemea
kuongezeka kidogo na kufikia kiasi cha shilingi za kitanzania bilioni 537, hatua ambayo
inapingana na dhamira ya serikali ya kudhibiti matumizi yasiyo ya lazima.

Protect yourselves against cholera: Wananchi, September 2010

Citizens should be on high alert and guard against cholera following an outbreak of the
obnoxious disease claimed four in Tarime recently.
According to media reports, doctors coordinating an anti-cholera campaign in the district
revealed that the number of cholera patients in hospitals around the district had increased from
20 to over 100 as of 25 August 2010.
Sikika advises citizens to keep their environment clean and observe high level of hygiene as this
will go a long way in protecting them against cholera. The general public should also adopt other
preventive measures such as boiling water before drinking or treat it with chlorine or iodine.
They can also resort to other safer beverages such as tea and coffee and carbonated bottled
beverages with no ice.

Health sector challenges not a license to corruption, August 2010

Health officials should shun corruption and do their work diligently despite challenges they
might face.
This comes amid reports that expectant mothers at Nyamongo village in Tarime District are
complaining over corrupt practices shrouding services at Sungusungu Health Centre at the
village. In response to the allegations, the medical officer in-charge of the health centre
highlighted that the center is facing staff shortage problem.
Corruption seems to have become a common practice in public health services. This impression
is supported by a snap survey conducted by Sikika in 2007, which covered all districts of Kibaha
and Dar es Salaam. It revealed that 21 percent of the 364 interviewed health workers have been
involved in corrupt behavior.

Stop using abusive language: Health Officials and Workers, July 2010

Medical practitioners should desist from using abusive language when addressing patients as it
agitates and discourages them from seeking medical attention.

Recently, it was reported by the media that Morogoro regional authorities addressed complaints
raised by patients who stated that health officials and workers in various facilities in the region
have been using abusive language to patients. Morogoro Regional Commissioner Issa Machibya
told nurses and doctors at a meeting that he received a lot of complaints in his office concerning
health service delivery and mistreating of patients.

Abusive language is against health ethics and it shows unwillingness thereby making patients
failing to perform their work passionately with courtesy as the Health Client Service Charter
states. Health service providers often abstain from communicating with service users, but Sikika
reminds them that communication is vital for the health profession because it influences
productivity and outcomes. Ineffective communication contributes to lost time,
misunderstanding, strained relationships, and inhibits future communication. It is even more
critical because it is not numbers, buildings, or sale targets that are the ultimate goal; it is people,
patients and their health. Good, bad, or nonexistent communication has an impact on the patients.

Wataalam wa Afya wameonywa kuacha kutumia lugha chafu wanapohudumia wagonjwa kwani kwa
kufanya hivyo kunaathiri tabia ya kutafuta matibabu na kuwakatisha tamaa wagonjwa wanaokwenda
kupata huduma katika vituo vya huduma za afya vya umma.

Hivi karibuni, Mamlaka ya Mji wa Morogoro ilifanyia kazi malalamiko ya wananchi juu ya watumishi na
wafanyakazi wa afya katika vituo mbalimbali vya huduma mkoani humo ambao wamekuwa wakitoa
huduma chini ya kiwango na kutoa lugha chafu kwa wagonjwa.Mkuu wa Mkoa wa Morogoro, Mh. Issa
Machibya amewaambia wauguzi na waganga katika mkutano kwamba amepokea malalamiko mengi
ofisini kwake kuhusu utoaji huduma za afya na unyanyasaji wa wagonjwa.

Lugha chafu kwa wauguzi huthibitisha mmomonyoko wa maadili ya kazi na ukosefu wa wito wa kazi
hivyo kushindwa kutekeleza majukumu yao kwa kutoa huduma kwa wagonjwa kwa upendo na
unyenyekevu kama ilivyobainishwa katika sera za serikali na Mkataba wa huduma kwa mteja. Sikika
inapenda kuwakumbusha watoa huduma hawa kuwa, mawasiliano ni muhimu hasa katika taaluma ya
afya kutokana na matokeo yake. Kutokuwepo na mfumo thabiti wa mawasiliano baina ya watoa na
watumia huduma kunachangia kwa kiasi kikubwa kupotea kwa muda, kutoelewana, mahusiano mabaya
na kukwamisha uboreshaji wa huduma. Katika huduma za afya, mawasiliano ni muhimu zaidi sababu
lengo kuu ni kujali wananchi, wagonjwa na afya zao na wala si namba, majengo ama mauzo ya huduma
hizo. Hivyo basi, mawasiliano mazuri, duni ama kutokuwepo kabisa mawasiliano kuna athari kubwa kwa
watumia huduma.

Government should intensify war on ghost workers – July 2010

The media is awash with reports about 2,924 ghost workers who have been milking from the
government and finally, have been removed from the payroll system. In fact, the government
incurred a loss of Tsh7 billion during 2008/9 financial year from these ghost workers.

According to the reports, the Minister of State in Prime Minister’s office (Policy, Coordination
and Parliamentary affairs), Mr Phillip Marmo said that an appraisal was conducted in the
education sector in 2007 and found a total of 1,413 ghost workers. In another review that was
done for the health sector in 2008, found 1,511 ghost workers.

The Controller and Auditor General (CAG) on the health sector for the year that ended in June
2009 indicates that a sum of Tsh 77,707,817 was paid to retirees, deceased and absconded
employees, which is unpractical and may result into nugatory expenditure of public funds. The
report from the prior financial year (2008) also indicates that there were salaries adding to Tsh
325,574,300 paid to retired, absconders and deceased and chances of recovery are remote.

Medical Practitioners should not mislead pregnant women to opt for the knife-July 2010

Medical practitioners should not mislead pregnant women to go for caesarean deliveries for their (doctors)
personal gain.

Some sections of the media recently reported that the number of Caesarean deliveries is escalating at an
alarming rate in this country owing to doctors misleading pregnant women into opting for the knife.
According to their investigations, doctors are encouraging pregnant mothers to opt for caesarean surgery
for profiteering purposes of which hospitals, especially privately owned, are said to be reaping handsome
returns from the growing trend.

The number of caesarean deliveries are said to have risen four times to a shocking 70 percent in a month
according to data from health insurance providers, which was shared with the media. The reports also
state that the payment for a caesarean surgery in middle class private owned hospitals is between Tshs 1.5
million and Tshs 2.5 million, while normal deliveries cost between Tshs 500,000 and Tshs 700,000.

Sikika discovered that though the media quoted recommendations by the World Health Organization
(WHO) that only 15 percent of all births should be done through the knife, the WHO recently dropped these
recommendations arguing that there was no evidence for placing a limit. The WHO now states that “there
is no empirical evidence for an optimum percentage


General Budget Support cuts: a case of shifting burden to citizens- June 2010

THE government should protect against misuse and abuse of public resources to safeguard the development
partners’ budget support contributions for the benefit of the ordinary citizens.
This follows a report from the development partners who agreed to reduce the commitment to the General
Budget Support (GBS) in Tanzania for the recently approved budget (FY 2010/2011) to $534 million. This
amount is $220 million less than the last financial year 2009/2010 commitment.
The development partners announced this in a press release dated on Thursday 13 May, 2010. It not only had
the government running around to look for other sources of funds, but also shifted the burden to ordinary
citizens.
According to the press release, one of the major reasons for this reduction is the slow implementation of reform
programmes, thereby making it difficult to achieve the expected outcomes. Resulting from slow pace of
implementation, many targets like reduction of maternal mortality rate from the current 578 to 265 per 100,000
live births by 2010 (as per the National Strategy for Growth and Reduction of Poverty) are far reaching.
Government’s objectives have been difficult and take long to accomplish mostly because allocation of funds has
not been efficient and effective across all Ministries, Departments and Agencies. A good example is from the
budget books for the year 2008/09 and 2009/2010, where the amount allocated for seminars and related items
stand at Tshs 683,747,329,980 and Tshs 530,337,691,576 respectively. As health activists, we find it unworthy
to spend such a huge amount of money on unnecessary items like these while we have more demanding areas in
the primary health services to concentrate on.
Misuse and abuse of public resources should stop immediately. Under the current financial situation, the
government must improve its performance for the better of the country. Evidence of such issues is documented
in the reports of the Controller and Auditor General. An example of this is found in the audit report for the
Ministry of Health and Social Welfare for the year ended June 2008 and June 2009 of which Tshs. 325,574,300
and Tshs.77, 707,817 respectively were paid to the so called ‘ghost workers‘.
Sikika therefore, feels that the government should avoid unnecessary suffering of citizens by making sure that
transparency and accountability is up held when it comes to the allocation and implementation of budgets in
order not to shift the burden to citizens.
